Colorado State University – Pueblo’s Internal Auditing department has established this web access form to provide the campus with a mechanism to ask the auditor questions, provide the auditor with information about concerns that you may have regarding university activities or make comments about the audit process. There may be times when an employee has a concern but does not know who to talk to. Internal Audit encourages university employees to first consider discussing their concerns with their supervisor. If you would rather report a matter to Internal Audit, we will help you start the process of responding to your concern.
When you submit information using the web form, you can choose whether to send your personal information such as name and contact information. We encourage you to include contact information, so we can follow up with you to get additional information or clarify your report, but if you choose not to share your contact information with us then your input will be anonymous (and we will not have a way to contact you with information regarding our follow-up). Internal Audit will handle all information received in a confidential and discrete manner. It is expected that any comment or concern provided to Internal Audit will be submitted by CSU-Pueblo’s faculty, staff or students in good faith.
