CSU-Pueblo is pleased to offer an accident and sickness insurance plan for students. Participation is not mandatory but is recommended.
The web site below will allow students to access the benefit plan and insurance enrollment form: www.eciservices.com .
Student Health Services will also have enrollment forms and brochures available. Payment can be made by check, money order, or credit card at the college. The last day to enroll is listed at the top of the Benefit Plan.
Make the check or money order payable to “ECI” in U.S. dollars or refer to the “Charge Card Authorization Payment Information” box listed at the bottom of Enrollment Form. Mail this enrollment form along with premium payment postmarked no later than the “Last Day to Enroll” date above to ECI, Box 264, Jefferson, CO 80456.
